 To initiate an investigation an Assignment Sheet must be completed. The Assignment Sheet allows McGough & Associates to collect detailed information regarding the request. Please make every effort to provide as much detail as possible for those fields that are applicable. McGough & Associates also offers background checking services. To request a background check, please complete the Background Check form.
